| Women More Prone to Facial Wrinkles |

A study by a team of plastic surgeons from the Netherlands, published in Aesthetic Surgery Journal, suggests that differences in the skin around the mouth (perioral area) account for more, and deeper skin wrinkling in women than in men.
The study’s goal was to 1) determine whether or not women wrinkle earlier and more severely than men, and 2) to develop better strategies for the treatment of facial wrinkles.
The study found several contributing factors for more perioral wrinkles in women:
- Women have fewer sweat glands and sebaceous glands than men, which influences the natural filling of potential wrinkles.
- The skin around a woman’s mouth contains fewer blood vessels than men, which may speed wrinkle development.
- Women have tighter muscle to skin attachment around the mouth then men. This may cause more of an inward pull to create deeper wrinkles.
Current treatments for perioral wrinkles include the use of ActiveFX and DeepFX lasers, Botox, and injectable wrinkle fillers such as Juvederm, Restylane, Radiesse, and other methods such as Dermabrasion, and chemical peels. |

Keeping up the fitness and diet regimen during winter can be a challenge for some. Here are a few simple tips that can help get you through in style:
- Lift your Spirits – Physical activity can lift your spirits during limited sunlight and it can insure you’re in good shape for spring or for a getaway south.
- Dress properly – Make sure the layer closest to your body is made of a breathable, moisture wicking material. Insulate extremities, and be sure to wear a hat.
- Take time to warm up and cool down – Cold weather constricts muscles so you have to allow enough time to get the muscles warm and functioning.
- Turn adversity into opportunity – Don’t let the winter blues extend to your feelings about exercise. Be creative. Cross country skiing, or snowshoeing are great aerobic activities. Take a class like yoga, or pilates that you’ve been meaning to try.
- Maintain good eating habits – It’s easy to fall into the routine of unconscious eating during football games and evening television. Keep up good eating habits. Now that peaches, watermelon, strawberries and other summer fruits are out of season, turn to apples, pears, pomegranates, cranberries, kiwi, grapefruit, and oranges to satisfy that sweet tooth.
- Find tasty veggies – A great winter produce choice is squash. There are a variety of winter squash available – acorn, spaghetti, butternut, pumpkin are just a few.

| Are Dark Circles Under your Eyes Making you Look Older? |

I’m only 40, and my eyes look like my grandmother’s! What can I do?
Non-surgical treatments can be amazing for many areas, but the eyes are one area of the face where results without surgery like blepharoplasty are minimal in more advanced cases for baggy eyelid skin, or dark circles.
Dark Circles Under the Eyes
Dark circles under the eyes can be their own special joy. While many products say they work to correct dark circles, this is only true in limited situations. The causes for dark circles under the eyes are actually quite varied. These include diet, heredity, dehydration, a loss firmness in soft tissue structures, and a decrease in bone mass of the bone under the eye.
Eye Creams
The staff and I are often asked for recommendations on a great eye cream. While a high quality eye cream such as iS Clinical’s Eye Complex, our Firming Eye Cream, or Obagi Elastiderm Eye Cream can be great for correcting puffiness, fine lines and wrinkles, and in some cases, even minimizing dark circles depending on the cause, most are not. I have yet to come across a store bought formula that works well for dark circles under the eyes.
A Treatment that Works
While a blepharoplasty is one of the single best treatments for saggy eyelids, a blepharoplasty alone may not be enough to correct dark circles under the eyes. A total eyelid rejuvenation may be the best way to completely revitalize the areas around your eyes.
This three-step approach blends laser blepharoplasty (eyelid surgery), Arcus Marginalis Release, fat grafting, or Tear Trough Implants (depending on the exact cause of the dark circles), and a TotalFX treatment to totally revitalize the areas around your eyes. | Melanoma on the Rise, but Preventable |

I recently read an article in the Wall Street Journal on the rising instances of Melanoma, which is the most serious form of skin cancer. Since we’re heading into the summer months, I wanted to share some of the pertinent parts of the article.
While cancer rates for many forms of the disease are falling, melanoma rates are actually rising. In fact it’s now the most common form of cancer for adults 25 to 29 years of age.
Although only 3% of all skin cancers are Melanoma, this type of cancer causes over 75% of deaths from the disease. However, there is good news. If a tumor is caught before it actually penetrates the outer layer of the skin, the survival rate is about 99%. As you can see, early diagnosis and treatment are critical.
Dr. Darrell Rigel, a clinical professor of dermatology at New York University Medical Center has identified 6 factors (using data from 300 patients with the disease and 300 control patients) that can help predict melanoma risk. These include:
- Blistering sunburns as a teenager
- Outdoor jobs for 3+ years as a teen
- Red or blond hair
- Marked freckling on the upper back
- A family history of melanoma
- A history of “actinic keratoses” – are dry, scaly, lesions that are an early stage of skin cancer
Two or more of these risk factors increases your risk of melanoma 5 – 10 times over the general population’s.
The American Academy of Dermatology recommends checking moles using a tool dubbed the ABCDEs of melanoma:
- Asymmetry – one half is unlike the other half
- Border – irregular, scalloped or poorly defined
- Color – varies from one area to another
- Diameter – size of a pencil eraser or larger
- Evolving – changes in size, shape or color over time
If you think you have any of these signs, contact your primary care physician, or your dermatologist. They’ll be able to accurately diagnose and treat any skin cancer related issues. And, remember, sunblock is a necessity during the summer months. |

| Don’t Believe the Hype About “Botox Breast Lifts” |

Recently there have been several stories in the media discussing a “Botox Breast Lift.” This non-surgical procedure is highly speculative, and has offered incredibly mixed results for the patients who have undergone this treatment.
Botox works in preventing wrinkles by weakening the muscle that has created the wrinkle in the first place. The principle here is similar. Essentially, Botox is used to weaken the pectoralis minor muscle (one of your chest muscles), which pulls the breast down. This causes the rhomboid muscle (a muscle at the top of your back) to pull up your back slightly, potentially giving the appearance of lifting your breasts, and also putting more pressure on your back.
The procedure requires three injections on each breast, and proponents say that it lasts approximately 10 – 12 weeks. However, injecting Botox into the chest muscles is not only dangerous, but . . .

| E-Botox Alternative – Relaxed Expressions |

Relaxed Expressions is one of the newest, minimally invasive, non-toxic tools in our ongoing quest to find the most effective methods for treating facial wrinkles, and reversing the signs of aging.

| Can I Have Multiple Procedures at the Same Time? |

I’ve received several questions from patients lately on whether or not they can have multiple procedures done during the same visit.
The short answer is yes, you can. However, the real limiter is the type of anesthesia selected.
Take liposuction for example. We often do smaller areas of liposuction under a local anesthesia with oral sedation. This method allows us a smaller window of time, so larger procedures aren’t possible. However, we can do larger areas under a general anesthesia. This also allows us to do combinations of surgeries.
A recent patient wanted to have liposuction and breast augmentation done at the same time as part of a “Mommy Makeover.” While we only did those two surgeries for that patient, I’ve had patients choose to have liposuction, breast augmentation with a breast lift, and a lower lid blepharoplasty. So there are fewer limitations under general anesthesia.
Having multiple procedures done at the same time is actually a smart choice for most of us. There is usually a shorter overall recovery time, a lower overall cost, and in many cases, we’re finding that combination treatments actually provide better results.

One of the things that we’re discovering is that a combination of treatments is best for treating the changes that occur with aging. | Lasers Reduce Swelling |

The use of lasers in cosmetic surgery has revolutionized the way both surgical and non-surgical treatments are delivered, increased the quality of the outcomes, and helped decrease recovery times.
I recently read an interesting study in the Archives of Facial Plastic Surgery that draws the conclusion that lasers, at least certain types, also help reduce bruising.
The study focused on the Vbeam Laser by Candela, and the results were actually stunning. In fact, the study showed as much as a 63% improvement within 24 – 72 hours in the type of bruising that occurs following a cosmetic procedure. These are dramatic results!
Many of the surgical and non-surgical procedures we perform at The Adams Center have bruising as a side effect. That bruising will usually go away within two weeks for a surgical procedure, often in less time for a non-surgical treatment.
To be able to reduce the swelling and recovery this much is a tremendous advantage, and will allow patients to be back to their daily activities more quickly.

| Combination of Facial Treatments Produce Results |

Our face is the most visible thing others see on a daily basis. As we age, our face is often the area that looks oldest first. But, what contributes to premature facial aging and what makes us look old? More importantly, can we reverse the process?
Several things can contribute to facial aging. The obvious answer is “age”. While that’s very true, environmental factors, diet, chemicals we use on or around our face, whether or not we moisturize, weather, smoking, and a host of other factors also contribute. So what makes us look old? Wrinkles around our eyes, mouth, forehead, etc. are often cited, but some of the other signs of an aging face include sagging skin, age spots, redness, and more.

- Make time for yourself – Get a facial, a massage, a manicure, or whatever your personal “me time” may be, take the time for yourself, and do it!
- Keep exercising – At the very least take a brisk walk every day to clear your thoughts, and get you blood pumping. This will not only release endorphins that give you that “runner’s high,” but will also help you feel less guilty about any extra calories you may be eating during the season.
- Set realistic goals – Baking cookies for the entire neighborhood is not necessary.
- Keep a list and prioritize – We can’t do everything all at once. It’s true at work, and it’s true for home and holiday planning as well. Make a list of all of the things that need to get done. Be realistic, and prioritize them using

| New Mammography Standards May Be Harmful |

Last week a group of advisors called the U.S. Preventive Services Task Force issued new guidelines for women to get mammograms. In it’s advisory capacity, the task force contradicted previous findings, and said that women in their 40s should not get routine mammograms for early detection of breast cancer.
Essentially the group raised the suggestions for the starting age of routine mammography by 10 years to age 50. The group recommended routine mammography screenings every two years for women ages 50 – 74. Risks and benefits for women age 75 and above are unknown, it said.
It’s important to note that the task force is composed of 16 health care experts, hand picked by the White House, none of whom are oncologists. The group reviews medical data and bases recommendations on effectiveness and risks involved.
Breast cancer is the most common cancer for women in the U.S. Approximately 200,000 women are diagnosed with invasive breast cancer every year, according to the American Cancer Society. About 15 percent of women in their 40s detect breast cancer through mammography, many other women experience false positives, anxiety, and unnecessary biopsies as a result of the test, according to data.
These findings are at odds with the American Cancer Society, which looked at the same data but came to a different conclusion. “Breast cancer is a serious health problem facing adult women, and mammography is part of our solution beginning at age 40 for average-risk women,” it says. It recommends annual exams beginning at that age.
In my opinion, these guidelines are the first step in moving toward healthcare rationing, and reduced care to offset costs. One of the dangers in accepting these recommendations is that insurance coverage of mammograms could be dropped based on the new findings.
The most important thing for all women is to consult with their primary care physician, and have a frank discussion about breast cancer risks. Your risks may be higher due to family history of breast cancer, a concerning self exam, or other factors. So discuss the issues surrounding a mammogram, and make an educated decision on screening with your doctor.

| Reversing the Aging Process Using Hi-Tech, Low-Tech, the Tried & True |

Over the past few years, we’ve seen tremendous advances in high-tech anti-aging procedures. While it sounds cliché, these really have revolutionized the battle against wrinkles.
2006 statistics from the American Society for Aesthetic Plastic Surgery (ASAPS) showed an overall increase in cosmetic procedures. However, while surgical procedures declined, non-surgical procedures increased to account for 83% of the total cosmetic procedures performed in the U.S.

| 11.7 Million Cosmetic Procedures in 2007 |

People deciding to enhance their appearance increased slightly to 11.7 million (both surgical and non-surgical) in 2007.
Women have accounted for about 90% of total procedures for some years now, and this was true again in last year. Women had 91% of the cosmetic procedures. But, men are also realizing the benefits of a cosmetic procedure. Men who decided to enhance their appearance through a cosmetic procedure increased significantly (17%) from 2006.
Both surgical (up 8%) and non-surgical procedures (up by 1%) increased last year:

- Botox Treatments, with 2,445,656 treatments, it was by far the most common cosmetic procedure
- Restylane , and Juvederm combined to account for 1,364,533 procedures
- Laser Hair Removal – 1,226,974
- Microdermabrasion – 743,748
- IPL/Photofacials rounded out the list with 584,530 procedures.

| A Dramatically Improved Facelift for Our 40′s |

The last few years have brought about some terrific advances in plastic surgery. Better techniques, more effective procedures, and combinations of procedures have all helped to improve our ability to turn back the clock.
Once we hit our mid to late 40′s, aging begins to become noticeable in the mid-face region. There are several great non-surgical treatments at this point that can reduce wrinkles and delay the clock for most people.
Until recently, women in their 40s and 50s wouldn’t have considered a facelift as an option. I receive questions from patients and prospective patients on a daily basis. The following have been asked recently (more or less the same question) by several people.
Q: How realistic are non-surgical procedures? For example will I notice a definite improvement in my wrinkles?
A: Good question! Not every treatment works well for every patient. We spend time during the consultation to do a thorough analysis to determine which will work best for your situation. When selected properly, patients will see definite improvement.
Q: I would like a breast enlargement, but I was doing some research and found that they are not permanent, and I would have to replace the breast implants in a certain amount of time. Is this true? How long do they last? Would I lose sensitivity with an implant replacement?
A: Breast implants are not permanent, but currently carry a 10 year warranty. Typically, a breast implant replacement would be done at some point after that, but that is an individual decision.
Regarding your question on loss of sensitivity – based on patient feedback, there really isn’t a loss of breast sensitivity in my experience.
